Objective: to determine the incidence of postoperative pulmonary complications in patients with Atrial Septal Defect and Ventricular Septal Defect and to determine the risk factors for postoperative pulmonary complications in these group of patients.

Study Design: prospective cohort

Patients: Atrial Septal Defect and Ventricular Septal Defect , 6 -19 years old with preoperative 2D-echocardiogram and pulmonary function test results and underwent open heart surgery

Measurements and Results: significant risk factors associated with postoperative pulmonary complications are: abnormal nutritional status ( moderate to severe wasting) (p=0.043); presence of respiratory tract infection prior to surgery (p=0.038); elevated pulmonary artery pressure> 50 mm Hg (p=O.OOI); Qp:Qs > 1.5 : 1 ( p=O.OOI); mean FVC 69% predicted (p=0.003); mean FEVl 62% predicted ( p=0.009) and mean FVC + FEVlIFVC 151 (p=0.006).

Conclusion: patients with congenital heart disease with left to right shunts have some lung dysfunction and at increased risk of developing pulmonary complications postoperatively. Early identification and institution of prophylactic measures may help prevent and lessen the complications.
