Thyroid nodules and masses are some of the most commonly encountered neck lesions on diagnostic imaging, especially in CT. A radiologist who comes across these thyroid lesions may recommend various strategies for further assessment, such as suggesting further imaging, clinical or laboratory evaluation, ignoring, watchful waiting, or any combination of these. The most common recommendation taken is that of further imaging, typically with ultrasound. While present recommendations are partly-based on statistical evidence regarding cancer risk of thyroid nodules, most are ultimately derived from expert opinion. Thus, whether follow-up of such nodules truly results in patient benefit remains unclear.

Performing sonography for all thyroid nodules incidentally detected using non-ultrasound cross-sectional imaging such as CT is not cost-effective and could be for some of the patients, especially those in the marginalized sector. It could subject the patients to undue stress and financial burden from the uncertainty of additional work-up.

If certain CT imaging features of incidentally detected thyroid lesions is found to be highly correlated with certain TI-RADS scores and their corresponding recommendations, then reporting such CT findings would save time and money directing the clinician to the appropriate work-up for the patient, i.e., ultrasound monitoring or direct histopathologic evaluation. The findings in this study may also help radiologists in crafting reporting guidelines and recommendations regarding thyroid nodules and masses detected on CT, as the present decision algorithms are often based on ultrasound imaging features.

There have been prior studies describing the CT imaging findings of thyroid nodules as well as comparing the diagnostic utility of CT for thyroid nodules and masses versus histopathology. However, this study is the first to directly correlate CT imaging features with ACR TI-RADS scores.
