Background: Currently, ischemic stroke is one of the leading causes of mortality during the past decade. Medical treatments among high-risk patients are quite costly and sometimes may even require surgical procedures, which are beyond the financial capacity of most Filipinos. A safe and effective alternative is currently needed. Objective: To determine the presence of in vitro thrombolytic activity of 100uL normal saline-reconstituted chloroform extracts of Averrhoa bilimbi (Kamias) fruit using in vitro clot percentage clot lysis method. Study Design: The study was a controlled experimental trial. Study Setting: With permission, the study was conducted in the Biochemistry (for extraction) and Microbiology ( for incubation) Laboratories of the Cebu Institute of Medicine (CIM). Study Population: With a 95% confidence level and 10%margin of error, the computed sample size from a population of 510 is n=81. Maneuvers: A chloroform extract of the Averrhoa bilimbi was prepared based on the in vitro model to study clot lysis percentage activity. The extracts were reconstituted using 100uL normal saline solution, and extracts were added to the preformed, weighted thrombi in sterile test tubes. After incubation at 37C for 90 minutes in a water bath, the fluid was removed, and the remaining thrombus weighed again. Outcome Measures: The percent lysis of the extracts were computed to determine if it reaches at least 10%; the proportion of samples showing significant thrombolytic activity were determined, and ANOVA was used to measure statistical significance. Results: There were a total of 60 blood samples taken from 20 healthy male volunteers. The mean percent clot lysis time for the samples with chloroform was 35.3559%. This was considered significant as it exceeded 10%. Out of 20 test samples, 15 showed percent clot lysis exceeding 10%. The percent clot lysis of the samples incubated with streptokinase was 66.6391%, while that of the samples incubated with saline was 0.0171%. One-way ANOVA performed on the means showed a sign value of 0.116, which was less than the p value of 0.05.
