The Research Institute for Tropical Medicine (RITM) performs the following functions as the reference center for the surveillance of multi-drug resistant typhoid fever (MDR-TF): 1. conducts training workshops for the participating laboratories emphasizing the proper identification of Salmonella typhi and performing antimicrobial susceptibility patterns (ASP); 2. confirms the identity and ASPs of bacterial isolates referred by participating laboratories; 3. performs minimum inhibitory concentration and other special tests on referred isolates; and 4. coordinates with the appropriate foreign institutions for referral of bacterial isolates for molecular typing studies.

From August 1993 to the present, the RITM has accomplished the following for the MDR-TF surveillance.

1. It has conducted a laboratory training workshop on October 28-29, 1994 in accordance with the first function mentioned above. It was participated in by 17 laboratories. 2. It has received a total of 346 bacterial isolates for confirmation. 3. It has performed disc diffusion tests and minimal inhibitory concentration on the referred isolates.

RITM has also coordinated with Brown University, Rhode Island, U.S.A., for the performance of molecular typing on some of the epidemic strains and some of the sensitive isolates. (Author)

Objectives

The study aims to improve treatment of multidrug-resistant typhoid fever (MRD-TF) and recommend specific control measures to prevent spread of endemic MDR-TF.The specific objectives are: 1) to sustain an existing hospital-based surveillance system for MDR-TF in Metro Manila; 2) to determine resistance rates of S. typhi and paratyphi A to various antimicrobials used in the treatment of typhoid fever; 3) to establish a bank of drug-resistant isolates for further study; 4) to provide specific recommendations on therapy and control measures to prevent transmission and spread of MDR-TF.
