High performance liquid chromatography (HPLC) determination of serum penicillin in Filipino children with rheumatic fever/rheumatic heart disease (RF/RHD).

Rheumatic Fever (RF) and Rheumatic Heart Disease (RHD) in the Philippines remain to be significant causes of morbidity and mortality among children. This study aimed to determine serum levels of benzathine penicillin G (BPG) in Filipino children with RF/RHD after intramuscular administration of 1.2 million units of benzathine penicillin G after 21 and 28 days using the HPLC. Levels were determined at the following time intervals: 0,7,14,21 and 28 days after the administration of BPG. Thirty two subjects with serum levels were recruited from the Department of Pediatrics, Outpatient Department-Philippine General Hospital. Peak serum levels were noted on day 14 (8.70 ug/ml) which declined on day 21 (7.69 ug/ml) and returned to baseline values on day 28 (5.15 ug/ml). No significant change was noted on day 14 and day 21 but highly significant decrease from day 21 and day 28 (p≤0.05). Patients on day 21 had BPG serum concentrations >0.03 ug/ml in 90.7% and 59.4% on day 28. This trend seems to suggest that patients should indeed receive BPG every 21 days based on the pharmacokinetics of the drug.
