Two dimensional echocardiography in infective endocarditis: A diagnostic tool and therapeutic guide.

Bacterial endocarditis often presents a diagnostic challenge and until the advent of echocardiography, there was no available non-invasive test that permitted visualization of the characteristics valvular vegetation. The introduction of echocardiography has provided a superior imaging procedure to visualize and study vegetative lesions in vivo. In 1972, Schelbert and Mueller reported the echocardiographic evidence of infective endocarditis. Subsequent reports confirmed the usefulness of echocardiography in the diagnosis and management of patients with infective endocarditis Before the introduction of two dimensional echocardiography (2D-echo), the M-mode technique was used for the diagnosis of infective endocarditis. The characteristic M-mode appearance of vegetative lesion is due to the non-uniform echo producing mass attached to the valve or annular area. Terms like shaggy, fuzzy, fluffy or soft echoes have been coined by various observers. Unlike M-mode imaging, 2D-echocardiography permits the localization and recognition of the form, size and motion of masses attached to valves, cardiac and vascular walls. This report illustrates 3 cases of infective endocarditis whose vegetations were imaged by 2D-echo. The usefulness of 2D-echo in the diagnosis and management of patients with infective endocarditis is discussed in this communication.
