Aging is a highly individual process that affects people in different ways. Elderly are expected to experience changes in the level of functional health in  their body characterized by the presence of disabilities and impairment in function, hence the need for proper caregiving and health management. This study dealt on the profile of the caregivers of the elderly, and of the elderly in the first, second and third districts of Albay, the leading diseases of the elderly and the care given to them. Results revealed that the caregivers belonged to middle years of their lives majority of whom are female. Most of the elderly were 66-71 years old, had varoius eductaional attainment with majority of them as elementary graduates. Most of these elderly were dependent on their children, and on their pension as source of financial support. leading diseases were hypertension follwed by osteoarthritis, diabetes mellitus and pressure ulcer. Care strategies provided to the elderly were in the form of medication, food, physical care needs, and emotional and psychological care. The module deloped to gyide the caregivers and family members is recommended.
