Early tracer uptake of multiple renal cysts in a paediatric case of unilateral hydronephroureter during diuretic 99mTc_diethylene triamine pentaacetic acid (DTPA) renography.

Abstract

The paucity of functioning renal parenchyma in polycystic kidneys results in poor to non-visualization on radionuc/ide renography. We report the case of a 4-year old male who presented with recurrent urinary tract infection and left hydronephroureter with multiple renal cysts on ultrasound. Diuretic 99mTc_DTPArenal scan showed gradual tracer accumulation in multiple distinct cystic structures in the left kidney with eventual tracer washout in the post-void images with and without diuresis. Visualization of distinct "grape-like" structures with radiotracer concentration that exhibits tracer washout on dynamic renography can therefore differentiate communicating polycystic conditions from megaca/yces.
