Pulmonary functions in Filipino patients with type 2 diabetes mellitus: A preliminary study .

BACKGROUND: Periodic monitoring of lung function has been advocated as a measure of health status and a prognostic indicator of premature death from all causes. The association of reduced lung function with other known diabetes related end organ damage has been observed by number of studies. However, spirometric PFTs in other reviews failed to show differences between patients with diabetes and normal control subjects.

OBJECTIVES: (1) To determine if there is a difference in pulmonary function between Filipino patients with Type 2 DM and non-diabetics; (2) to describe the baseline characteristics of the subject population in terms of age, sex, body mass index(BMI) and glycosylated hemoglobin (HbAlC); and (3) to determine if there is a correlation between diabetes, spirometric measures and glycemic control .

METHODOLOGY: This is a cross-sectional study where pulmonary function test (PFT) of 73 patients with Type 2 diabetes for at least 5 years and 70 non-diabetics from May to October 2005 were compared at St. Luke's Medical Center. The main outcome measures were forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), FEV1/FVC ratio and HbA1c.

RESULTS: The FEV1 and FVC mean values were significantly lower among diabetics, 77.8 percent (p 0.001) and 69.9 percent (p<0.001) respectively, compared to non-diabetic group,85 percent and 77.9 percent respectively. Using logistic regression, the odds of having abnormal FEV1 among diabetics is 3.13 (95 percent CI 1.41-6.94) times higher than the non-diabetics (p 0.005), the odds of having abnormal FVC is 2.219 (95 percent Cl 1.0-4.92) times higher than non-diabetics (p 0.050). HbAlc is inversely correlated with FEV1.

CONCLUSION: Reduced pulmonary function is seen in Filipino patients with Type 2 DM. The severity of which appears to be related to glycemic exposure.
