Endoscopic variceal rubber band ligation using novel transparent multi-band (six-shooter) deployed.

Endoscopic Variceal Ligation (EVL) using a novel transparent multi-band (six-shooter) deployer, was performed in 12 cirrhotics who had upper gastrointestinal hemorrhage from ruptured esophageal varices. Banding sessions were done once weekly until all varices were eradicated. Attempts to ligate all visualized varices at every EVL session were aggressively pursued. A total of 116 bands were deployed in 20 EVL sessions. Complete variceal eradication was achieved in all 8 patients. Five patients required 2 sessions for eradication, 2 patients needed only one session while another had 3 EVL sessions for complete eradication. There were no endoscopy-related complications during the procedure. Two Child's Class C patients died on the 3rd and 4th hospital day due to end stage cirrhosis and exsanguination, respectively. One patient was lost to follow-up. This novel multi-band (6-shooter) deployer is a major advance in esophageal band ligation of bleeding varices. Rapid multiple band deployment without the need of an overtube, less scope re-insertions, a transparent shaft for better visualization, shorter duration of EVL session and satisfactory variceal eradication makes this new innovation a gem of a gadget.
