Correlating diabetic autonomic neuropathy using R-R interval variability with ventricular arrhythmias and dysfunction.

The syndrome of autonomic neuropathy in diabetes occurs at a sub-clinical stage which could predispose to sudden death and cardiac arrest among diabetics. Mortality rate associated with this has been reported to be 50% in 3 years. Attempts to determine the presence of cardiac autonomic neuropathy (CAN) among diabetics are made in this study and correlating this with the occurrence of and predisposition to ventricular arrhythmias and the presence of cardiac dysfunction. There were 34 subjects, 18 diabetics and 17 non-diabetics on whom we performed treadmill exercise test (TET) with deep breathing substituted for hyperventilation in the protocol, Holter monitoring, signal averaged electrocardiogram (ECG), and 2-dimensional echocardiogram (2-D echo). There were 9 who tested positive in the CAN test, mostly females with mean age of 63. Chi square analysis did not show any relation with regard to the presence of autonomic neuropathy and occurrence and predisposition to arrhythmias and that of cardiac dysfunction. This test can be utilized at the bedside and for ambulatory patients in defining a high-risk group of diabetics especially the asymptomatic subset.
